STEP 4 - The CareBrigade provides day-to-day support to patient.

  • During the life of the CareBrigade, the CarePartner checks in regularly with the patient (and live-in caregiver if there is one), by phone, email or in person, to see what is working and what isn’t. Then, in consultation with the patient, the CarePartner makes necessary adjustments.
     
  • If needed, the CarePartner, communicator, scheduler, and/ or brigade coordinators can email volunteers with any adjustments to the patient's predicted needs or if there is a request for additional skills and/or additional volunteers.
     
  • Any volunteer changes through the at-home recovery period should be made through the CarePartner, caregiver or brigade coordinator, not the patient.
